The Inspector is the mistake meter. He rises out of the floor when you serve an anomaly, refuse a human, or fail a prompt. The 2AM Inspection is a different event: an empty black car and a walk out the back door.

What raises him
- Serving an anomaly, refusing a real customer, or failing the clown or Wendigo prompt. Fandom and IGN agree on those triggers.
- No source I checked gives a number of mistakes per phase. I am not inventing one.
- Look: tall, red-brown mushroom-like head with a chunk missing on the right, no eyes or mouth, long black coat, boots, and gloves. That description is Fandom's.

Internal Control costs 5,000 on both Fandom and TechWiser. Fandom says it resets him to phase one. TechWiser says it clears mistakes only when he is about to come out. Inspector versus the 2AM Inspection
Around 2AM a black car with no driver stops and the screen edge goes red. Leave through the back door. Do not look back, do not watch the cameras, and do not look at the Inspector. Looking back is the Erased ending on IGN. TechWiser once called the inspection weekly; IGN, Fandom, and Gaming Route say it is nightly. I lead with nightly and keep the weekly line as a single-source disagreement. The empty car is what people mean by driverless car. It is not a customer anomaly.
